RESUMO

Indole is often associated with a sweet and floral odor typical of jasmine flowers at low concentrations and an unpleasant, animal-like odor at high concentrations. However, the mechanism whereby the brain processes this opposite valence of indole is not fully understood yet. In this study, we aimed to investigate the neural mechanisms underlying indole valence encoding in conversion and nonconversion groups using the smelling task to arouse pleasantness. For this purpose, 12 conversion individuals and 15 nonconversion individuals participated in an event-related functional magnetic resonance imaging paradigm with low (low-indole) and high (high-indole) indole concentrations in which valence was manipulated independent of intensity. The results of this experiment showed that neural activity in the right amygdala, orbitofrontal cortex and insula was associated with valence independent of intensity. Furthermore, activation in the right orbitofrontal cortex in response to low-indole was positively associated with subjective pleasantness ratings. Conversely, activation in the right insula and amygdala in response to low-indole was positively correlated with anticipatory hedonic traits. Interestingly, while amygdala activation in response to high-indole also showed a positive correlation with these hedonic traits, such correlation was observed solely with right insula activation in response to high-indole. Additionally, activation in the right amygdala in response to low-indole was positively correlated with consummatory pleasure and hedonic traits. Regarding olfactory function, only activation in the right orbitofrontal cortex in response to high-indole was positively correlated with olfactory identification, whereas activation in the insula in response to low-indole was negatively correlated with the level of self-reported olfactory dysfunction. Based on these findings, valence transformation of indole processing in the right orbitofrontal cortex, insula, and amygdala may be associated with individual hedonic traits and perceptual differences.

BACKGROUND: Noninvasive brain stimulation (NIBS) techniques are a promising tool for treating the negative symptoms of schizophrenia. Growing evidence suggests that different dimensions of negative symptoms have partly distinct underlying pathophysiological mechanisms. Previous randomized controlled trials (RCTs) have shown inconsistent impacts of NIBS across dimensions. OBJECTIVE: This systematic review and meta-analysis evaluated the effects of NIBS on general negative symptoms, and on specific domains, including blunted affect, alogia, asociality, anhedonia, and avolition. DATA SOURCES: PubMed, Web of Science, Embase, Cochrane CENTRAL, PsycINFO, OpenGrey, and Clinicaltrials.gov from the first date available to October, 2023. RESULTS: Among 1049 studies, we identified eight high-quality RCTs. NIBS significantly affects general negative symptoms (SMD = -0.54, 95% CI [-0.88, -0.21]) and all five domains (SMD = -0.32 to -0.63). Among dimensions, better effects have been shown for improvement of avolition (SMD = -0.47, 95% CI [-0.81, -0.13]) and anhedonia (SMD = -0.63, 95% CI [-0.98, -0.28]). Subgroup analyses of studies that applied once daily stimulation or >10 sessions showed significantly reduced negative symptom severity. CONCLUSION: NIBS exerts distinct effects across multiple dimensions of negative symptom, with treatment effects related to stimulation frequency and total sessions. These results need to be confirmed in dedicated studies.

Chemosensory disorders (including smell, taste and chemesthesis) are among the established symptoms of COVID-19 infection; however, new data indicate that the changes in chemosensory sensation caused by COVID-19 may differ among populations and COVID-19 variants. To date, few studies have focused on the influence of the SARS-CoV-2 Omicron variant on qualitative changes and quantitative reductions in chemosensory function in China. We conducted a cross-sectional study of patients with COVID-19 caused by the Omicron variant, to investigate the prevalence of chemosensory disorders and chemosensory function before and during infection, using an online questionnaire. A total of 1245 patients with COVID-19 completed the survey. The prevalence rates of smell, taste, and chemesthesis disorders were 69.2%, 67.7%, and 31.4%, respectively. Our data indicate that sex, age, smoking, and COVID-19-related symptoms, such as lack of appetite, dyspnea, and fatigue, may be associated with chemosensory disorders during COVID-19. Self-rating of chemosensory function revealed that patients experienced a general decline in smell, taste, and chemesthesis function. Further longitudinal research studies are needed to generate additional data based on objective assessment and investigate the factors influencing chemosensory function in COVID-19.

The comprehension of metaphor, a vivid and figurative language, is a complex endeavor requiring cooperation among multiple cognitive systems. There are still many important questions regarding neural mechanisms implicated in specific types of metaphor. To address these questions, we conducted activation likelihood estimation meta-analyses on 30 studies (containing data of 480 participants) and meta-analytic connectivity modeling analyses. First, the results showed that metaphor comprehension engaged the inferior frontal gyrus, middle temporal gyrus, fusiform gyrus, lingual gyrus, and middle occipital gyrus-all in the left hemisphere. In addition to the commonly reported networks of language and attention, metaphor comprehension engaged networks of visual. Second, sub-analysis showed that the contextual complexity can modulate figurativeness, with the convergence on the left fusiform gyrus during metaphor comprehension at discourse-level. Especially, right hemisphere only showed convergence in studies of novel metaphors, suggesting that the right hemisphere is more associated with difficulty than metaphorical. The work here extends knowledge of the neural mechanisms underlying metaphor comprehension in individual brain regions and neural networks.

Purpose: Most studies on olfactory function in individuals with bipolar disorder (BD) have not distinguished between the different subtypes or between the acute phase (mania or depression) and euthymic state. In this study, we compared olfactory function among BD patients with different subtypes and episodes to explore the potential use of olfactory function as a biomarker for the early identification of BD. Patients and methods: The study sample consisted of 117 BD patients who were hospitalized between April 2019 and June 2019, and 47 healthy volunteers as controls. The BD patients were divided into a bipolar I disorder (BD I) (n = 86) and bipolar II disorder (BD II) group (n = 31) according to the different subtypes, and divided into depressive BD (n = 36), manic BD (n = 44), or euthymic BD (n = 37) groups according to the types of episodes they experienced. We assessed olfactory sensitivity (OS) and olfactory identification (OI) via the Sniffin' Sticks test and used the Hamilton Depression Rating Scale (HAMD) and Young Manic Rating Scale (YMRS) to evaluate BD characteristics among all subjects. Results: Compared with controls, the participants with BD showed decreased OS and OI. We found statistically significant differences in OS and OI between the BD I group and controls, as well as differences in OS between the BD I and BD II group. Least-significant difference multiple comparisons revealed statistically significant differences in OS between the depressive BD group, manic BD group and controls and also between the manic BD and euthymic BD group. OI was positively correlated with the YMRS score in the BD I group and OS was negatively correlated with the HAMD score in the BD II group. Conclusion: This may be the first study to compare olfactory function in patients with BD I vs. BD II via pairwise comparisons. Our findings suggest that OS may have potential as a biomarker for distinguishing the different subtypes of BD and as a state-related biomarker for differentiating the acute phase from the euthymic state of BD. However, further prospective research is warranted.

BACKGROUND: Odor memory is an important field of clinical research for its distinctive characteristics, which differ from those of other sensory systems. To date, several tests have been implemented for the assessment of odor memory. Despite a range of studies demonstrating the importance of verbal mediation in odor memory, few have distinguished odor memory performance in different odor verbalization levels. NEW METHOD: We aimed to develop a standardized odor memory test toolbox with one group of odors that are easily verbally identified and the other group of odors that are difficult to identify. The test contained two odor categories (high- and low-verbalizability odors), each consisting of three subtests (short- and long-term memory and working memory). RESULTS: Satisfactory test-retest reliability and solid validity of the odor recognition and working memory test were shown in both odor categories. Moreover, people scored significantly better with high- than low-verbalizability odors. A negative age effect on odor memory performance was also found. COMPARISON WITH EXISTING METHODS: No previous odor memory test distinguished odor memory performance in different odor verbalization levels, while the Olfactory Memory Test Battery (OMTB) contains high- and low-verbalizability odors and each category has three subtests. CONCLUSIONS: The present study indicated the OMTB is a comprehensive assessment of odor memory with good reliability and validity. All subtests can be used separately or in combination with each other according to the clinical and research needs.

Olfactory sensations contribute to sexual desire and sexual behavior. However, the degree to which individual importance of olfactory function and body odors relate to sexual desire is not known. This study was conducted to preliminarily examine these relationships among Chinese college students (N = 1903) via the Importance of Olfaction Questionnaire, the Body Odor Sniffing Questionnaire, and the Sexual Desire Inventory, which were used to measure subjective significance of olfaction, frequency of sniffing self or others, and sexual desire, respectively. Individuals who assigned higher importance to olfaction or engaged more in body odor sniffing showed stronger sexual desire. We further explored these associations in different cultures to determine whether cultural consistency existed. We conducted a second study to make cross-cultural comparisons between Indian (N = 313) and US (N = 249) populations. For both countries, a higher importance placed on olfaction and a higher prevalence of body odor sniffing were consistently associated with stronger sexual desire. In conclusion, our study confirmed that people who placed more value on olfactory function or engaged more in body odor sniffing showed stronger sexual desire. These correlations were consistent for both sexes and across different cultures, further indicating the importance of olfaction in sexuality.

OBJECTIVES: Body odor can convey much information about an individual and thus we frequently engage in sniffing one's own and other people's body odor. However, there is scarce evidence on the within- and cross-cultural variation in body odor sniffing behaviors and no psychometric scale for specifically measuring such behaviors. Hence, our study aimed to develop the Body Odor Sniffing Questionnaire (BOSQ) and used it to make a cross-cultural comparison. METHODS: In Study 1, 2,026 participants were recruited from our university, with one half used for exploratory factor analysis (EFA) to examine the factor structure of the BOSQ (sample 1) and the other half used for confirmatory factor analysis (CFA) to verify the factor structure (sample 2). In Study 2, 352 Chinese and 254 US participants were recruited to complete the BOSQ through Wenjuanxing and Amazon Mechanical Turk, enabling comparison of body odor sniffing behaviors across two cultures. RESULTS: The Study 1 results showed that the BOSQ comprises 17 items in three factors: self-private body odor, others' body odor, and self-common body odor. The CFA results further supported that this three-factor model was a good fit. The Study 2 results showed that US participants scored higher overall and on the self-private body odor and others' body odor dimensions, whereas Chinese participants scored higher on the self-common body odor dimension. CONCLUSIONS: The BOSQ demonstrated good reliability and validity, which is a useful tool for evaluating individuals' body odor sniffing behaviors. Cross-cultural difference existed as the US population reported a higher prevalence of body odor sniffing behavior, compared to the Chinese population.

Background: Previous studies have investigated the relationship between schizotypal traits and odor identification ability as well as the relationship between schizotypal traits and odor hedonic capacity in adults. However, very little is known about the relationship among these three factors, especially in children and adolescents. The current study aimed to explore the relationship among these three factors in children and adolescents as well as the potential role of odor identification ability. Method: A total of 355 non-clinical children and adolescents (aged 9−16 years) were recruited in the study. They were asked to complete the Universal Sniff Test (U-Sniff), the Chemosensory Pleasure Scale for Children (CPS-C), and the Schizotypal Personality Questionnaire­Child (SPQ-C). Results: The SPQ-C scores were negatively correlated with both the odor identification scores and the odor hedonic scores (p = 0.022 and p < 0.001, respectively). Only the interpersonal−affective factor of the SPQ-C was negatively correlated with the odor identification scores (p = 0.031). The odor identification scores were significantly positively associated with the odor hedonic scores (p < 0.001). Moreover, the relationship between odor hedonic capacity and schizotypal traits, especially the interpersonal−affective factor, was mediated by odor identification ability. Conclusions: Schizotypal traits were negatively correlated with both odor identification ability and odor hedonic capacity in children and adolescents, while odor identification ability was found to mediate the relationship between odor hedonic capacity and schizotypal traits, especially the interpersonal−affective factor. Our study indicated that improving odor identification ability through olfactory training may have a positive influence on odor hedonic capacity in individuals with schizotypal traits.

OBJECTIVES: Dreams often involve visual and auditory sensations, but olfactory experiences have not received the same amount of attention. This study explores the prevalence and content of olfactory dreams in China and investigates the relationship between olfactory imagery, olfactory significance, and olfactory dreamers. METHODS: In the first part of the study, 4302 dream records from LOFTER were screened and classified to preliminarily identify the prevalence and content of olfactory dreams. In the second part, 718 participants completed an online questionnaire about olfactory dreams, imagery, and significance. RESULTS: The prevalence of olfactory dreams in the diary dream entries and questionnaire survey participants were 3.95% and 18.70%, respectively. Instances in which odors appeared in dreams were more positive than negative and were mainly related to food, burning and smoke, body odor, nature, and certain environments and objects. Moreover, individuals with olfactory dreams showed better olfactory imagery, and stronger olfactory significance. CONCLUSIONS: Olfactory sensations occurred in the dreams of Chinese individuals, but their prevalence was very low. Most of the odors that emerged in these dreams were ones that the dreamers were familiar with in their daily lives.

BACKGROUND: Early-onset obsessive-compulsive disorder (EOCD) is a comparatively severe subtype of obsessive-compulsive disorder (OCD). Olfactory dysfunction is a common symptom of OCD, but all previous studies have focused on late-onset OCD (LOCD). METHODS: The current study compared olfactory identification ability in EOCD patients and age-matched and sex-matched LOCD patients and healthy controls. Thirty patients with EOCD, 30 patients with LOCD and 30 healthy controls were included in the study. Olfactory function was measured using the University of Pennsylvania Smell Identification Test. The Logical Memory and Visual Reproduction components of the Revised Wechsler Memory Scale were used to evaluate verbal and visual memory. RESULTS: There were significant differences in olfactory identification ability between the three groups. EOCD patients were comparable to LOCD patients, while both patients' group showing worse olfactory identification ability than controls. Olfactory identification ability was not significantly correlated with verbal and visual memory or clinical symptoms in the EOCD group or the LOCD group. CONCLUSIONS: The results of the present study suggest that olfactory identification ability may be a relatively stable indicator of OCD, independent of age, duration of illness, verbal and visual memory, and severity of clinical symptoms.

OBJECTIVE: Olfactory deficits have been reported in bipolar disorder, but this finding is controversial. This study investigated whether olfactory deficit can serve as a specific marker for bipolar disorder by comparing olfactory function in different mood episodes of bipolar disorder. We also compared olfactory function in bipolar disorder and other mental disorders - namely, major depressive disorder and schizophrenia. METHODS: The study consisted of two experiments. Experiment 1 enrolled 175 bipolar disorder patients (70 depressed subgroup, 70 manic subgroup and 35 euthymic subgroup) and 47 controls. Experiment 2 enrolled the participants from Experiment 1, along with 85 major depressive disorder and 90 schizophrenia patients. The Sniffin' Sticks test was used to evaluate odour identification ability and odour threshold (as a measure of odour sensitivity). The Hamilton Depression Rating Scale and Young Mania Rating Scale were used to assess depressive symptoms in all subjects and manic symptoms in bipolar disorder patients, respectively. We also used the Positive and Negative Syndrome Scale to assess clinical symptoms in schizophrenia patients. RESULTS: All three bipolar disorder patient subgroups (depressed, manic and euthymic subgroup) showed reduced odour identification ability compared to controls; however, only patients in the acute phase of a mood episode (depressed, and manic subgroup) showed impaired odour sensitivity. Clinical symptoms were negatively correlated with odour sensitivity but not odour identification ability. Bipolar disorder and major depressive disorder patients showed less odour identification and sensitivity impairment than schizophrenia patients. CONCLUSION: Odour sensitivity is a potential dopaminergic marker for distinguishing between bipolar disorder patients in acute phase vs remission, while odour identification is a trait but a nonspecific marker of bipolar disorder.

Olfactory impairment and rapid eye movement sleep behaviour disorder (RBD) are prodromal symptoms of Parkinson's disease (PD) that may be associated with each other. This review aims to investigate the significance of olfaction in the diagnosis and prognosis of patients with RBD and to assess moderating factors affecting olfactory performance. We searched articles on olfaction in RBD and PD in five electronic databases. We identified 32 studies for the systematic review and used 28 of those, including 2858 participants for meta-analysis. Results revealed significant deficits in odour identification (g=-1.80; 95% CI: -2.17 to -1.43), threshold (g=-1.29; 95% CI: -1.67 to -0.91), discrimination (g=-1.08; 95% CI: -1.28 to -0.87) and overall olfactory function (g=-1.64; 95% CI: -1.94 to -1.35) in patients with RBD. Except for the Unified Parkinson's Disease Rating Scale Part III scores, none of the known moderating variables (including age, sex, disease duration and years of education) accounted for the olfactory function heterogeneity in patients with RBD. We identified similar olfactory impairments in patients with RBD and patients with PD (either with or without underlying RBD). These findings suggest that olfactory impairment may be a sensitive and stable diagnostic biomarker of RBD and appears to be useful for identifying patients with idiopathic RBD at high risk for early conversion to PD.

BACKGROUND: "Early-onset schizophrenia" (EOS) is defined as disease with onset before the age of 18 years. This subset of schizophrenia exhibits worse cognitive function and carries a worse prognosis than adult-onset schizophrenia (AOS). Olfactory impairment has been found in patients with schizophrenia-spectrum disorders. However, most research has focused on olfactory impairment in patients with AOS: olfactory function in EOS is not known. The aim of this study was to investigate the olfactory identification ability in EOS, and its relationship with negative symptoms. METHODS: We compared olfactory function between two independent samples: 40 patients with EOS and 40 age- and sex-matched healthy controls (HCs); as well as 40 patients with AOS and 40 age- and sex-matched HCs. The University of Pennsylvania Smell Identification Test was administered. RESULTS: The EOS group and AOS group exhibited worse olfactory identification ability than HCs; impairment correlated significantly with negative symptoms. Olfactory identification was worse in patients suffering EOS compared with those suffering AOS. CONCLUSION: Olfactory identification impairment may be a trait marker of schizophrenia.

Anhedonia, the loss or decline of the ability to enjoy pleasure, is an important clinical characteristic of schizophrenia. Schizotypal traits refer to the appearance of subclinical symptoms of schizophrenia across normal people. Still, few studies have investigated chemosensory anhedonia in schizophrenia patients and schizotypy individuals. Seventy-one schizophrenia patients (SCZ), 162 schizotypy individuals (SCT) as selected by the Schizotypal Personality Questionnaire (SPQ), and 182 healthy controls (HC) participated in our study. We used the Positive and Negative Syndrome Scale (PANSS) to measure the clinical symptoms of schizophrenia patients. All participants completed the Chemosensory Pleasure Scale (CPS), which was used to assess participants' smell and taste hedonic capacities. We found that the three groups differed in chemosensory anhedonia. The SCZ group presented more severe chemosensory anhedonia than the SCT group, and the SCT group presented more severe chemosensory anhedonia than the HC group. We also found that chemosensory hedonic capacity was negatively correlated with negative schizotypal traits in the SCT group. Our results suggested that chemosensory anhedonia is an important characteristic of schizophrenia spectrum disorders.

Researchers have examined if olfaction is a sensitive biomarker of anorexia nervosa, but considerable heterogeneity across studies makes it difficult to reach a consensus. This review and meta-analysis sought to clarify if olfaction is altered in individuals with anorexia nervosa and explore potential moderators of olfaction in this population. We performed quantitative and qualitative analyses of olfactory function in individuals with anorexia nervosa compared with healthy controls. A random effect model was used to estimate pooled effect sizes, and meta-regression was conducted to identify potential moderators. We found that individuals with anorexia nervosa had largely intact olfactory function compared with healthy controls in terms of threshold (g = -0.09, 95% confidence interval [CI] (-0.65,0.47), p = 0.757), identification (g = -0.06, 95% CI (-0.32,0.20), p = 0.642), and overall olfactory function (g = -0.47, 95% CI (-1.02,0.07), p = 0.090). Discrimination was different from control (g = -0.51, 95% CI (-0.97,-0.05), p = 0.029). However, after sensitivity analysis, the pooled effect size was nonsignificant in discrimination. Olfactory sensitivity covaried with anorexia nervosa severity, body mass index (BMI) positively moderated olfactory threshold score (ß = 0.79, 95% CI (0.18,1.41), p = 0.020) in individuals with anorexia nervosa. Disease duration negatively moderated olfactory threshold score (ß = -0.21, 95% CI (-0.40,-0.03), p = 0.034). The results suggest that olfaction is not a sensitive marker of anorexia nervosa diagnosis, but olfactory sensitivity may be a useful indicator of anorexia nervosa severity.

A body of research has investigated the relationship between alexithymia and sleep problems, as well as the relationship between schizophrenia and alexithymia. However, there have been few studies on the relationships between the three. The current study explored the relationship between schizotypal traits and sleep problems among college students, and the potential role of alexithymia as a mediator of this relationship. The participants were all first-year students at a medical university in Guangdong province, China. A total of 2,626 college students participated. They were asked to complete a questionnaire that incorporated the Schizotypal Personality Questionnaire (SPQ), the Toronto Alexithymia Scale (TAS-20), and the Insomnia Severity Index (ISI). The results revealed a relatively high percentage of students with mild insomnia (74.8%) and a smaller percentage with moderate to severe insomnia (7.9%). Correlation analysis revealed that both the TAS-20 and ISI scores had significant positive correlations with the SPQ score (p < 0.01). There was also a significant positive correlation between the TAS-20 and ISI scores (p < 0.01). The ISI score was significantly influenced by the SPQ score in a direct way, and increased considerably with increases in the TAS-20 score, indicating the importance of alexithymia as a mediator. The mediation model was tested via regression analysis and the bias-corrected bootstrap method, and these results further confirmed the role of alexithymia as a mediator.

Specific anosmia is defined as the inability to detect a particular odorant, despite a normal olfactory function. Previous studies reported sex-related difference in detection threshold to steroid odorants, like androstenone or androstadienone during adolescence, and boys showed an increased detection threshold with age. However, such investigations have not been performed for non-steroid odorants. Hence, the current study investigated sex- and age-related effects on the prevalence of specific anosmia in children/adolescents aged 5-14 years (n = 800) to non-steroid odorants. The detection thresholds of three non-steroid odorants (bacdanol, methylsalicylate, and 3-hydrox-2-methyl-4-pyrone) with different molecular weights were measured. Results indicate that the rate of specific anosmia for all three odorants decreases from childhood to adolescence. This decrease is independent of sex and is most pronounced for odorants with higher molecular weight compared to the odorant with lower molecular weight. Thus, the development of basic olfactory functions for these three odorants continues until adolescence. The effect of molecular weight suggests that the locus of effect is at the periphery of the olfactory system, due to changes in either olfactory receptor expression patterns or perireceptor events.

OBJECTIVES: Olfactory dysfunction is a common problem. However, too little attention has been paid to questionnaires used to evaluate quantitative olfactory dysfunction. Therefore, the current study aimed to develop a simple self-reported Mini Olfactory Questionnaire (Self-MOQ) for the screening of quantitative olfactory dysfunction in clinical practice. METHODS: Two hundred and eighty-five patients who had subjective complaints of olfactory disorder participated. The Sniffin' Sticks test score was used to define functional anosmia, hyposmia, or normosmia. We assessed the factor structure as well as internal consistency, convergent validity, and discrimination performance. RESULTS: The results showed that the final version of the Self-MOQ included only one factor with five items. The Self-MOQ has a good internal reliability (Cronbach's α = 0.84) and validity (r = -0.60, P < 0.001). The receiver operating characteristic analyses indicated that the Self-MOQ as compared to a visual analogue scale (VAS) is an effective measure for discriminating normosmic from hyposmic/anosmic patients, anosmic patients, and hyposmic patients. CONCLUSION: The Self-MOQ is a simple, reliable and valid questionnaire to screen olfactory dysfunction in clinical practice that appears to be superior to the use of VASs but does not replace olfactory testing.
